Summer Intensive Information and Forms
EMIA 2020 Student Handbook
Below is the link to the 2020 EMIA Student Handbook. Please read the handbook carefully. Most questions about the intensive will be answered in the handbook. Additionally there are forms below that MUST be completed and emailed to steffen@diversityofdance.org or mailed to EMIA, 2 Merry Acres Lane, New Milford, CT 06776. All forms should be completed and returned by June 30th. If for any reason forms cannot be completed by that date, please contact steffen@diversityofdance.org.
Forms to Complete:
Medical Forms
The following forms MUST be printed and signed and returned, preferably via email to steffen@diversityofdance.org.

Youth Camp Health Exam MUST be printed and signed by physician. **Note: the health exam form may be valid for ONE year only.

Authorization for Self-Administration of Medicine by Camper MUST be printed and signed by physician in order for students to carry and selfadminister their medications (in two places). Please fill out ONE form PER medication to be self-administered. Note, we must have a form for each medication whether a prescribed OR over-the-counter medication.

Insurance Card Form MUST be printed and name filled in. Then front and back of insurance card must be copied or scanned.

Meningoccal Meningitis Vaccination Form MUST be signed by parents and emailed or mailed with other medical forms.

EMIA 2019 Video Order Form
You may complete the order form for DVDs, BluRay Disks, or individual dance links and scan and email it to info@emiadance.org or bring the completed form to any EMIA performance on July 13, 20, 27 or August 3. No orders will be accepted after August 10th. Payment when emailing may be may be made by mailing a check made out to EMIA and mailed to EMIA, 2 Merry Acres Lane, New Milford, CT 06776 or by clicking Donate on the website (make sure the student name is in the memo section). Orders dropped off at a performance may be paid for with a check or cash.
